Form I-693 | Report of Medical Examination and Vaccination Record

Purpose of Form

According to the section 212(a)(1) of the Immigration and Nationality Act, there are some medical circumstances, which can be a reason for inadmissibility of people who want to become owners of green cards. For example, the list of these circumstances includes the availability of dangerous and communicable diseases, which considered being dangerous in the US. I-693 Form shows the results of this medical examination and information about previous vaccinations. 

Special Instructions

If you want to get a green card and show that you don’t have these diseases, you have to submit USCIS Form I-693 (Report of Medical Examination and Vaccination Record). This form has to be completed by a USCIS-certified doctor. If this form has already been sealed by the surgeon, you should not open it.

Common filing mistakes

  • Using an outdated USCIS edition of the form.
  • Leaving required fields blank or mixing typed and handwritten answers inconsistently.
  • Forgetting the signature, supporting evidence, or the correct filing fee.

Where to file: After completing your immigration medical examination, the civil surgeon must give you the completed and signed Form I-693 in a sealed envelope. Do not accept the Form I-693 if it is not in a sealed envelope. You (not the civil surgeon) must submit your Form I-693 to USCIS.

Hello, I did not know that it is impossible to unpack an envelope with the results of a medical examination. In conclusion, everything is in order, but the envelope is opened. Does it make sense to attach it to the form? Or you need to go through the inspection again? Thanks!
