Form I-693 | Report of Medical Examination and Vaccination Record
Purpose of Form
According to the section 212(a)(1) of the Immigration and Nationality Act, there are some medical circumstances, which can be a reason for inadmissibility of people who want to become owners of green cards. For example, the list of these circumstances includes the availability of dangerous and communicable diseases, which considered being dangerous in the US. I-693 Form shows the results of this medical examination and information about previous vaccinations.
Special Instructions
If you want to get a green card and show that you don’t have these diseases, you have to submit USCIS Form I-693 (Report of Medical Examination and Vaccination Record). This form has to be completed by a USCIS-certified doctor. If this form has already been sealed by the surgeon, you should not open it.
