Download Form G-639 Freedom of Information Act / Privacy Request. The form is used to request access to information in accordance with the Law on Freedom of Information and Privacy. PA allows US citizens or legal permanent residents to:
1. Get access to information related to them in the records of the federal agency;
2. Have a copy (full or partial);
3. Correct or change notes;
4. Allow individuals to make inquiries about which records related to them are collected, used or distributed.
PA also prohibits disclosure of information about any person, without his (her) written consent, except for certain situations.
