Download form N-648, medical certification for persons with disabilities. When applying for US citizenship, you must demonstrate a certain level of knowledge of English (written, oral), knowledge of history and the principles and forms of the US government. You need to fill out form N-648 if you want to get an exception from the testing requirements due to physical or mental disability. Form N-648 must be sent along with the application for the naturalization of N-400. Fee for filing form N-648 is not charged.
