Download the I-765 application form for a work permit. The form is used to obtain a work permit document (EAD) and social security number. Some foreign nationals in the United States may file Form I-765 "Application for a Work Permit" to request a work permit and a work permit document (EAD). Other foreign nationals whose immigration status permits them to work without restriction in the United States can also use Form I-765 to apply to the United States Citizenship and Immigration Services (USCIS) for EAD.
